Enrollment Information

Summer Enrollment:
BHS will enroll new students on the following dates with an appointment:
June 6, 13, 20, 27
July 24, 25, 26, 27, 31
BHS will enroll new students with an appointment or walk-in on the following date:
August 2

In order for your student to be enrolled at Brookwood High School, the student and parent/legal guardian (with photo ID) must be present.

The following documentation must be provided at time of registration.

Proof of Guardianship (one of the following):
    • Birth Certificate
    • Guardianship papers from Gwinnett County Justice Center Probate Court
770-822-8265 (if you are not the natural parent with custody).

Proof of Residency (one of the following along with a current utility bill, electric, gas or water:
    • A non-contingent sales contract (must close within 30 days)
    • Current lease/rental agreement
    • Most recent income tax return
    • Current paycheck stub
    • Current residential property tax statement or bill
    • Current warranty or quick claim deed
    • Current home purchase agreement (settlement statement)
    • Current homeowner’s insurance policy
    • Third person notarized affidavit of residency. (Must get affidavit form from Counseling or Main Office).
  • Withdrawal form from the previous school showing date and classes/grades at the time of withdrawal.
  • Official/Unofficial Transcript and test scores showing high school coursework.
  • Discipline Record providing complete history of discipline.
  • Attendance Record providing complete history of attendance (including prior year).
  • Immunization Record (Georgia Form 3231) and Hearing, Vision and Dental Screenings (Georgia Form 3300). Out of state students only will be granted a 30 day extension.
  • Social Security Card (optional)
  • Special Education – Copy of student’s IEP (Individual Education Plan) if applicable.
We welcome you to pick-up the enrollment packet prior to your appointment at the Counseling or Main Office. This will expedite your enrollment time. 


The counseling office looks forward to working with you to enroll your student at Brookwood High School. You will need to allow approximately one hour for the enrollment process.
